The client’s chest was crushed between his seat and the steering wheel and has a tension pneumothorax. He has also sustained a fracture of his T1 and has a partial severance of his spinal cord at that level. His upper and lower extremities are mottled in appearance with weak peripheral pulses; his JVP (jugular venous pressure) is high. Blood pressure is 86/50 mm Hg, and his pulse is 52 and regular. The client is difficult to arouse.
  1. For the nurse to adequately care for this client, she must have a clear understanding of which concepts of circulatory function?

b. What psychosocial influences or stressors would the nurse be considering for this client?


Answers

Patient has tension pneumothorax and circulatory insufficiency. Releasing tension pneumothorax is a priority. Subsequently, we should examine patient for bleeding. Maintenance of adequate perfusion is needed. One should perfuse adequately to avoid excess fluids causing pulmonary edema.

A neurogenic shock will also need reperfusion to begin with. Vitals should be monitored regularly including peripheral pulsation.

Psychosocial stressors for the patient will be becoming bed-ridden, unable to manage his or her own bowel and bladder, and unable to feel the four limbs and torso.
